Laparoscopic Two-Staged Hepatic Resection

A 27yo female presented with acute onset of RUQ pain. She was diagnosed with multiple hepatic adenomas in the left lateral segments and the right lobe, with hemorrhage.The patient underwent a completely laparoscopic left lateral segmentectomy. Following discharge and stable recovery, the right portal vein was embolized. A hand-assisted laparoscopic right liver lobectomy was done 6 weeks later to remove residual adenomas. The patient was discharge in three days. This case reveals a new approach to patients with multiple hepatic lesions who may otherwise require transplantation or multiple open resections for cure. A staged laparoscopic approach can minimize morbidity and hasten recovery.
