Gastric Volvulus Following Laparoscopic Heller Myotomy and Dor Fundoplication for Achalasia

An extremely rare complication of Heller myotomy with Dor fundoplication is gastric volvulus. We present a case of acute volvulus following myotomy. A 26 year old male post Heller myotomy and fundoplication presented with vomiting and abdominal pain. CT confirmed gastric volvulus. Urgent endoscopic decompression, laparoscopic reduction, hiatal repair with absorbable mesh and Dor fundoplication were performed. A post operative contrast study was normal. He was discharged on a liquid diet. His diet was advanced without problem. Gastric volvulus is a rare but life threatening complication following myotomy and fundoplication. It must be identified and treated expediently.
