ARTERY FIRST APPROACH PANCREATICODUODENECTOMY WITH PV-SMV RESWCTION AND RECONSTRUCTION FOR PANCREATIC HEAD CANCER

We performed extended Kocher's maneuver, No. 16b1/a2 station of lymph nodes were cleared at this step.Then the gastrocolic ligament from the transverse colon was divided and SMV was exposed. After stomach transected, No.12 nodes around the portal vein and artery were removed. Then, cholecystectomy and dissection of the hepatoduodenal ligament were carried out. The common hepatic duct was divided. After division of the jejunumand the splenic vein, No. 7/8/9/12 station of LNs were further cleared with celiac trunk and common hepatic artery skeletonized. After segmental PV-SMV resection, specimen together with retroperitoneal soft tissue were removed en-bloc.
