# 2075 CORRELATION OF MAGNETIC RESONANCE CHOLANGIO-PANCREATOGRAPHY WITH FUNCTIONAL ASSESSMENT OF PANCREATOJEJUNOSTOMY AFTER WHIPPLE OPERATION

This study was carried out to evaluate the clinical significance of magnetic resonance cholangio-pancreatography (MRCP) assessment of pancreato-jejunostomy in patients after Whipple operation. Patients and Methods 15 patients with peri-ampullar tumors (N=12) or chronic pancreatitis (N=3) having receive standard Whipple operation were enrolled in this study. All patients had their reconstruction of biliary and pancreatic ducts as Child's arrangement. All patients had smooth post-operative courses and were re-assessed of the pancreato-jejunostomy by MRCP no earlier than one month after the initial Whipple operation. Clinical assessment of the functions of pancreatic exocrine was performed with symptoms, weight gain and two indirect pancreatic function tests: fecal elastase test and fecal chymo-trypsin test. Results There were 3 patients in whom the MRCP revealed pancreato-jejunostomy was stenotic with distal pancreatic ductal dilatation (N=2) or disappearance (N=1). All those three patients had their fecal elastase level <200 mg and fecal chymo-trypsin level <3U/g. Two patients were asymptomatic and one had failure on body weight gain and diarrhea. All the other patients with normal pancreato-jejunostomy on MRCP had their indirect pancreatic function test normal and were symptom free. Conclusion MRCP is a simple and accurate method for evaluating the anatomic and functional conditions of pancreato-enteral reconstruction after Whipple operation.
