Weight regain affects a significant proportion of post-bypass patients. Pouch and stoma dilation have been implicated as important factors in this process. The significant risk of morbidity with surgical revision makes endoscopic therapy an attractive treatment modality. This video reviews current endoscopic techniques including sclerotherapy, tissue anchor placement, and endoscopic suturing. Current evidence regarding these techniques is sparse, though compelling data are forthcoming. Given the growing bariatric population, endoscopic treatment of surgical failure will play an important role in the management of these patients.
