Mustafa Oruc*, Joshua Sommovilla, Emre Gorgun
Colorectal Surgery, Cleveland Clinic, Cleveland, OH

Introduction:
Hartmann reversal is a challenging procedure, especially in cases with dense adhesions, narrow pelvic anatomy,
Case Report:
A 52-year-old male with a history of diverticulitis perforation and failed Hartmann reversals underwent laparoscopic surgery. Adhesiolysis enabled access to the rectal stump. A stapled colorectal anastomosis revealed a leak, which was repaired with intracorporeal sutures under endoscopic guidance. A repeat leak test confirmed integrity, and a diverting loop ileostomy was constructed.
Conclusion:
This case highlights the integration of laparoscopic and endoscopic techniques to address complications, ensuring successful Hartmann reversal and optimal outcomes.
