We present here a 62-year-old female patient with advanced T2N0 anorectal lesion for which she received chemotherapy causing down staging of the lesion to T1 NO. After neoadjuvant therapy the patient was referred to us for possibility of endoscopic resection as she was refusing surgery. We did EID (endoscopic intermuscular dissection) & the lesion was removed enbloc. The pathology showed intramucosal cancer, no lymphovascular invasion or perineural invasion. EID had been shown to gain interest in the last years in anorectal cancers. This technique should be selected for special type of patients. EID helps in organ preservation & improving the quality of life for the patients.
