This 71-year-old female had recurrent rectal prolapse and fecal incontinence despite prior laparoscopic VMR (2014) and perineal repair (2021). She underwent laparoscopic suture rectopexy, where the detached mesh was left in situ and the rectum was fully mobilized and secured to the sacral promontory. Postoperatively, she had an uneventful recovery. At 1-year follow-up, she is free of prolapse with significant improvement in quality of life (Wexner score 18 to 12). This case demonstrates the feasibility and effectiveness of laparoscopic suture rectopexy for complex recurrent rectal prolapse, offering a safe and potentially durable solution.
