AdventHealth, Altamonte Springs, FL

29-year-old otherwise healthy woman presented with 1-year history of biliary fistula following a laparoscopic unroofing of a 10 cm multiloculated cyst. She developed postoperative fever and later infected biloma treated with 6-month IV antibiotics and multiple ERCP stenting along with percutaneous drain placements without resolution. Presenting MRI showed a 10.3 cm multiloculated cystic masses with thickened walls without clear communication with biliary tree. We herein showed succesful surgical debridement and operative management of persistent biliary fistula using a robotic platform, which should be considered an alternative solution to conventional open exploration.
