NAVIGATING LARGE CBD STONES INTRAOPERATIVELY: DIRECT VIDEO VISUALIZATION, EXPLORATION, AND RETRIEVAL

In this video, we present a 76-year-old male with a history of recurrent biliary pancreatitis and common bile duct (CBD) stricture who presented with one day of epigastric pain, obstructive bilirubinemia on labs, and concerns for acute cholangitis. His CT scan showed signs of acute cholecystitis, choledocholithiasis, and dilation of the hepatic and pancreatic ducts. After the initiation of IV antibiotics and resuscitation, the patient was taken for a robotic cholecystectomy, intraoperative cholangiogram, and CBD exploration using the SpyGlass direct visualization system. Two large CBD stones were successfully retrieved using a basket technique intraoperatively.
