Gallbladder carcinoma (GBC) is a highly aggressive malignancy often involving vascular structures. While controversial, laparoscopic radical surgery shows promise in selected advanced cases. A 53-year-old male with stage III GBC underwent laparoscopic resection with portal vein reconstruction. Pathology confirmed pT3N0M0 with no lymph node metastasis. After adjuvant chemotherapy, the patient achieved 23 months of recurrence-free survival. This case highlights the feasibility of laparoscopic surgery with vascular resection for advanced GBC and underscores the need for further studies to validate long-term outcomes and refine indications.
