CENTRAL HEPATECTOMY WITH IN-SITU VASCULAR RECONSTRUCTION: A CASE FOR FEASIBILITY AND ONCOLOGIC SAFETY

Centrally located liver tumors pose a challenge due to risks of post-hepatectomy liver failure and positive margins with non-anatomic resections. Central hepatectomy offers a balanced approach. This video demonstrates central hepatectomy for a large tumor involving the right anterior sector, segment 4, and middle hepatic vein, latter was reconstructed using a PTFE graft. Recurrent right portal vein thrombosis required intraoperative heparinization via an umbilical vein catheter, removed on day 5. The patient recovered uneventfully and continues to be disease-free at a 46-month follow-up. Central hepatectomy with vascular reconstruction ensures oncological safety and preserves liver function.
