PEPTIC PERFORATION OF THE ESOPHAGUS IN A PATIENT WITH ZOLLINGER-ELLISON SYNDROME

Peptic esophageal perforations are rarely seen in the current era of potent acid suppression medication. We review the surgical steps and postoperative management of a peptic esophageal perforation in a patient with Zollinger-Ellison syndrome associated with a pyloric neuroendocrine tumor. A 51-year-old woman presented to an outside facility with abdominal pain and melena. Endoscopy revealed duodenal bleeding ulcer and grade D esophagitis. CT showed pneumothorax and pleural effusion. She was transferred to our center, and we performed right VATS and noted a peptic esophageal perforation in distal esophagus. After decortication, a 19 Fr silicone T-tube was placed through the perforation.
