MULTIFACTORIAL DYSPHAGIA AFTER HELLER MYOTOMY IS BEST REMEDIATED WITH REVISIONAL HELLER MYOTOMY

This video highlights multiple factors contributing to dysphagia following Heller myotomy. The patient presented with hiatal stenosis, partially slipped fundoplication, and suspected inadequate myotomy, rendering them a suboptimal candidate for peroral endoscopic myotomy (POEM). We propose that such complex cases are best addressed through revisional Heller myotomy, which offers a more definitive and comprehensive approach to remediation of fundoplication and hiatal etiology of dysphagia.
