POSTERIOR RECTUS SHEATH FLAP FOR HIATAL AUGMENTATION (PORSHA) WITH DIAPHRAGMATIC RELAXING INCISIONS

Recurrence rates following paraesophageal hernia (PEH) repair remains a significant challenge in foregut surgery. Borrowing principles of myofascial advancement from plastic reconstructive surgery, the posterior rectus sheath flap for hiatal augmentation (PoRSHA) was developed for use in complex PEH repair. Here we demonstrate the PoRSHA technique along with diaphgragmatic relaxing incision in a patient with a large Type III PEH undergoing primary robotic PEH repair.
