ROBOTIC DISTAL GASTRECTOMY WITH LYMPHADENECTOMY AND GASTROJEJUNOSTOMY

This video is a robotic distal gastrectomy performed on a 78-year-old female. Initial findings included a hiatal hernia, circumferential ulceration, chronic gastritis, and mucinous adenocarcinoma with focal signet cell features. PET showed a hypermetabolic mass. Patient underwent four cycles of FLOT chemotherapy. The procedure involved extensive adhesiolysis, dissection of lymphatics and vascular structures, gastric transection with secure stapling, and anastomosis reinforced by sutures and sealant. Pathology revealed T1B poorly differentiated diffuse adenocarcinoma, with negative margins and no nodal involvement. The patient recovered well and was discharged on postoperative day three.
