CASE REPORT ON LAPAROSCOPIC LIGAMENTUM TERES CARDIOPEXY (LTC) AS A SUCCESSFUL TREATMENT FOR PATIENT WITH SEVERE GASTROESOPHAGEAL REFLUX DISEASE (GERD) COMPLICATED BY GRADE D ESOPHAGITIS POST SLEEVE GASTRECTOMY (LSG).

Background: Post-LSG patients often develop GERD and esophagitis due to weakened lower esophageal sphincter (LES). LTC aims to restore LES function by reinforcing esophagus with ligamentum teres.
Case Presentation: A 39-year-old male with refractory GERD and grade D esophagitis post-LSG failed medical therapy and lifestyle changes. Despite revision to Roux-en-Y gastric bypass, his symptoms persisted. He then underwent lap LTC and hiatal hernia repair.
Outcome: He showed significant improvement in symptoms, with esophageal healing on follow-up endoscopy. He discontinued proton pump inhibitors and remains symptom-free.
Conclusion: LTC can be an effective solution for refractory GERD post-LSG.
