LAPAROSCOPIC RIGHT HEPATECTOMY

We present a laparoscopic right hepatectomy in a 66-year-old male with a history of right renal tumor resection (2015). Follow-up imaging showed multiple hepatic cysts with calcific degeneration in segments IVa, V, VI, and VII, with no malignancy. Preoperative volumetry revealed a future liver remnant of 68%. Using five trocars, intraoperative ultrasound, and the Piggyback and Hanging Maneuvers, the liver was mobilized, and parenchymal transection was performed with ultrasonic energy. Vascular structures were secured, and the specimen was removed intact. Pathology confirmed benign cysts. The patient recovered uneventfully, was discharged on day five, and remains asymptomatic at six months.
