BILIARY CYSTADENOMA: LAPAROSCOPIC RESECTION & DIAGNOSTIC CHALLENGES

Biliary cystadenomas (BCA) are rare benign liver lesions with potential for malignant transformation. In case series, up to 80% of lesions initially diagnosed as BCA on imaging are ultimately found to be simple cysts on pathology. We present a laparoscopic resection of a suspected BCA in a 51-year-old woman with an enlarging hepatic cyst. Imaging revealed a complex cyst in the right lobe, with septations and areas of enhancement; however, pathology identified a simple biliary cyst. The video highlights the surgical approach to cyst resection and underscores that while BCA can be suggested radiologically, histopathological examination may reveal a benign simple cyst.
