Treatment of multifocal HCC is often challenging due to a potential 1% annual new tumor formation/recurrence within the cirrhotic background liver. We describe a minimally invasive robotic liver resection and ablation in multifocal HCC. A 70-year-old man presented with findings of HCC. Past medical history includes Hepatitis C. Workup included a CT and PET scan. CT scan showed three lesions, the largest in segment 3. Robotic parenchymal-sparing resection and ablation were performed. The estimated blood loss was 75 cc, and he was discharged on POD4. HCC tumors measured to be 6.6 cm and 5.8 cm with negative margins. At a 2-year follow-up, no evidence of cancer recurrence was seen.
