ROBOTIC SUBTOTAL PANCREAS HEAD RESECTION WITH LONGITUDINAL PANCREATICOJEJUNOSTOMY FOR CHRONIC PANCREATITIS

Minimally invasive surgery for chronic pancreatitis (CP) often focuses on pancreatic duct drainage rather than ensuring complete pancreatic head resection, which is crucial as the head is the pacemaker of pain. A novel approach ensures effective decompression of the head, body, and tail. Key steps include entry to the lesser sac, control of the gastrocolic trunk, suprapancreatic portal vein exposure, division of arteries, neck transection, and fluorescence-guided bile duct preservation. The pancreatic duct is fully opened, calculi removed, and a Roux limb created for lateral pancreaticojejunostomy. A separate enterotomy completes the anastomosis, offering effective pain relief.
