ROBOTIC PORTAL THROMBECTOMY DURING PANCREATODUODENECTOMY FOR ACINAR CELL CARCINOMA

Acinar cell carcinoma of the pancreas is a rare malignant tumor with better prognosis than ductal carcinoma. Surgical resection with negative margins should be pursued. We present a video of a 73-year-old man with a 7-cm ACC in the head of the pancreas with thrombus in the portal vein. MDT decided for upfront robotic resection. Operative time was 340 minutes. Venous reconstruction took 27 minutes. Recovery was uneventful and patient was discharged on PO day 6. Pathology confirmed ACC with free margins. Stage T3N1 (1/39). In conclusion, resection should be attempted in patients with ACC even in cases with a tumor thrombus. This procedure can be safely performed with the robotic platform.
