Here we presented a case with a diagnosis of an enlarging cystic lesion in the pancreatic head and recurrent acute pancreatitis. Preoperative imaging and testing suggested a mixed-type IPMN. We performed a robotic duodenum-preserving pancreatic head resection. The final pathology showed a gastric-type IPMN with low-grade dysplasia, involving main and side branch ducts, and a negative pancreatic duct margin. The patient recovered well and was discharged home on POD6 without complications. The video demonstrated the feasibility of a robotic approach to achieve adequate pancreatic head resection and efficient duodenum and common bile duct preservation.
