CENTRAL HEPATECTOMY WITH VASCULAR RECONSTRUCTION

Centrally located liver tumor is a surgical challenge traditionally treated with extended resections leading to a risk of post-hepatectomy liver failure. Central hepatectomy is a feasible choice in such patients with acceptable risks. This is demonstrated in a large tumor occupying the right anterior sector and segment 4 followed by vascular reconstruction. TVideo demonstrates the steps of central hepatectomy and reconstruction of the middle hepatic vein using a PTFE graft. Due to the recurrent thrombosis of the right portal vein, it was heparinized through a catheter left in the umbilical vein. It was removed 5 days later. Central hepatectomy is a feasible surgery with acceptable outcomes.
