Formal anatomical right hepatectomy is a major resection which can sometimes lead to hepatic insufficiency in patients with marginal future liver remnant function. An alternative technique via conventional open method proposed by Torzilli et al. demonstrates a less invasive approach using an ultrasound guided extended right posterior hepatic sectionetomy with preservation of right anterior inflow trunk. In this video, we describe a safe minimally invasive robotic technique for performing a parenchymal-sparing extended right posterior sectionectomy by including partial segment 5/8 resection with preservation of the inflow structures. The operation was unremarkable with minimal blood loss.
