ROBOTIC LEFT HEPATECTOMY AND THROMBECTOMY WITH PORTAL VEIN RECONSTRUCTION

Minimally invasive resection of liver tumors with vascular involvement is challenging and is not typically performed laparoscopically. Theoretical benefits of portal vein resection due to tumor involvement should be weighed against increased morbidity and mortality risks. This video demonstrates a pure robotic left hepatectomy with portal vein thrombectomy and reconstruction for hepatocellular carcinoma (HCC). A 69-year-old woman with a diagnosis of HCC on HCV-related cirrhosis presented with an 18 mm nodule in segment 4 close to the left portal vein with thrombosis on the preoperative CT scans. Liver function was preserved with Child-Pugh A5, ICG R15 of 9.6% and no portal hypertension.
