Perineal hernia is a rare complication of pelvic surgery. We present a case of a 75-year-old woman with a history of prior laparotomies for ovarian cysts, early rectal adenocarcinoma status post local excision and radiation (1999) and multiple transanal excisions for recurrent rectal tubular adenomas complicated by fecal/gas incontinence and rectal stricture. She underwent a robotic abdominoperineal resection and TAH-BSO in 2022. She presented 5 months later with a moderate perineal hernia containing small bowel. In this video we demonstrate perineal hernioplasty using a robotic transabdominal approach. This approach offers optimal access for dissection of hernia sac and mesh placement.
