Endoscopic Submucosal Dissection (ESD) offers several advantages in removing large, complex colon lesions. However, ESD is technically demanding and time-consuming due to insufficient endoluminal ability for traction-countertraction. Here we present an endoluminal procedure in a 55-year-old male with a 70 mm laterally spreading tumor in the ascending colon where we performed traction-assisted ESD facilitated by an external dynamic traction platform. The dissection was completed in 49 minutes without complication. The external dynamic traction platform can improve traction-countertraction and may decrease procedure time. Further studies are needed to evaluate efficiency and facility.
