RIGHT POSTERIOR BILE DUCT INJURY RECONSTRUCTION WITH ROBOTIC RY HEPATICOJEJUNOSTOMY

Right posterior bile duct injury with disconnected duct syndrome creates diagnostic challenges for many endoscopists and radiologists. The diagnosis and treatment are often delayed. In this video, we describe a discovery and minimally invasive treatment of right posterior bile duct injury in a young woman with Roux-en-Y Hepaticojejunostomy. An absence of contrast extravasation was seen during ERCP, however MRCP showed an isolated dilation of the right posterior hepatic duct with surrounding infrahepatic biloma. Operative exploration found a leaking right posterior duct requiring RY Hepaticojejunostomy reconstruction. The operation was uneventful and postoperative recovery was uncomplicated.
