Surgery, MetroHealth Medical Center, Cleveland, OH

45 yo M with walled off pancreatic necrosis who underwent robotic cystogastrostomy and transgastric pancratic necrosectomy. CT scan demonstrated necrosis confined to the lesser sac. A step-up approach was unsuccessful. Robotic debridement was offered. Internal drainage was achieved by the creation of a cystogastrostomy. A cholecystectomy was also performed. The patient is 2 months postop, doing well. Robotic pancreatic necrosectomy is a single stage, minimally invasive, potentailly more optimal solution in selected cases, for the surgical management of pancratic necrosis. The robotic platform overcomes limitations with visualization and handling that allow for safe transgastric debridement.
