We present a surgical case of laparoscopic repair of a slipped Nissen fundoplication; details of hiatus arch dissection, mediastinal dissection, and prior fundoplication takedown are highlighted. Interestingly, it was intraoperatively identified that the previous fundoplication was below an aberrant hepatic artery, which clearly demonstrates that the fundoplication had been incorrectly placed rather than having "slipped" after surgery. The video also shows a standardized technique to perform a compound partial fundoplication using an Angle of His accentuation (A.O.H fundoplasty) with Toupet fundoplication.
