REVERSAL OF TRANSORAL GASTROPLASTY USING TUNNELING ENDOSCOPIC METHODS

We present a case of weight regain in a 46-year-old woman with class III obesity following transoral gastroplasty. To allow for safe conversion to laparoscopic Roux-en-Y gastric bypass surgery, the request was made for endoscopic takedown of the vertical staple line to provide a patent proximal gastric pouch. Endoscopic tunneling methods were utilized, creating a mucosal lift with subsequent careful dissection of layers and cauterization of submucosal vessels. Muscular layers were dissected to release the vertical septum using electrocautery, resulting in a fully opened stomach. The patient subsequently underwent successful gastric bypass surgery with significant post-operative weight loss.
