A key oncological principle of rectal cancer surgery is en-bloc removal of the rectum and mesorectum around the avascular fascia propria. This is the total mesorectal dissection.
To appreciate this avascular plane, a broad traction and counter-traction is needed. Whilst retractor such as St Mark's retractor exist for open surgeries, there are no such retractors currently for minimally invasive approach. In this video, we describe five atraumatic retractors we have developed to optimise robotic and laparoscopic TME dissection. They are easy to make at the time of operation using available equipment in the operating room.
