In this video, we present an example of a robotic approach to locally invasive rectal using a case review. The patient is a 49-year-old woman with a history of metastatic rectal cancer with progression to near complete obstruction, vaginal invasion, and symptomatic rectal bleeding requiring multiple transfusions. She underwent robotic low anterior resection with en bloc resection of the proximal vagina and removal of the specimen through the vaginotomy. We hope to share technical knowledge that may permit surgeons to attempt complex multiorgan resections using a minimally invasive approach with the goal of decreased tissue trauma and more rapid postoperative recovery.
