ROBOTIC RESECTION OF HILAR CHOLANGIOCARCINOMA

A 76-year-old female with presented with progressive jaundice. Imaging showed a 1.8 cm tumor in the confluence of hepatic ducts. She was seen iniy considered not resectable due to arterial invasion. ERCP was performed and endoprosthesis was inserted. She underwent chemoradiation and after 5 months of treatment CT scan showed objective response. No portal or arterial invasion was seen. MDT decided for left hepatectomy (including caudate lobe) and Roux-en-Y hepaticojejunostomy. Robotic approach was proposed, and consent was obtained. Operative time was 8 hours. Estimated blood loss was 740 mL (Received 2 units). Pathology confirmed cholangiocarcinoma with free surgical margins T1aN0.
