LAPAROSCOPIC EPIPHRENIC DIVERTICULECTOMY AND HELLER MYOTOMY AFTER LAPAROSCOPIC NISSEN FUNDOPLICATION AND REVISION TO TOUPET FUNDOPLICATION WITH HIATAL CLOSURE WITH MESH

52 year old female with misdiagnosis of hiatal hernial status post Laparoscopic Nissen fundoplication, followed by revision with conversion to a Toupet Fundoplication and hiatus closed with mesh. She then presented to our practice a year later with continued reflux and dysphagia. Our work up revealed epiphrenic diverticulum with achalasia. She underwent Laparoscopic epiphrenic diverticulectomy and heller myotomy. Operative treatment of dysphagia should include a full work up for proper diagnosis and re-do laparoscopic approach to epiphrenic diverticulectomy is safe and feasible with experienced surgeons through meticulous dissection and proper operative planning.
