Surgical resection is the only hope of cure for pancreatic cancer, artery first and uncinate process first approach, are proposing aiming at improving R0 resection. Laparoscopy has flexible and unique view advantage from caudal, lateral posterior side, we describe a technique modification with emphasize a lateral and posterior dissection of pancreatic head as a novel approach for laparoscopic pancreaticoduodenectomy (PD). By flexibly taking view advantage of laparoscopy, lateral posterior approach will provide a good exposure of major vessels, help surgeons to completely dissect the vessel out of the lymphatic and nerve tissue under direct visualization in a comfortable way.
