Magnetic Sphincter Augmentation for Gastroesophageal Reflux Disease Management Following Laparoscopic Sleeve Gastrectomy

Laparoscopic sleeve gastrectomy (LSG) has been shown to decrease sequelae of morbid obesity including gastroesophageal reflux disease (GERD). However, new onset GERD after LSG can occur. This creates a considerable complication in patients were proton pump inhibitors (PPI) fail to control GERD. Surgical options are limited to conversion of LSG to Roux-en-Y gastric bypass. We present a case in which a patient with de novo GERD after LSG, refractory to aggressive PPI therapy, received off label placement of a magnetic sphincter augmentation device. Placement of this magnetic sphincter resolved GERD symptoms, suggesting a feasible and safe surgical option. Technique presented as follows.
