The first case describes a patient with a pancreatic cancer located in the neck of the gland treated by an extended pancreatectomy, lymphadenectomy and splenectomy. The second case is a young patient with insulinoma of the proximal body of the gland. She was treated by a vessel sparing, spleen sparing distal pancreatectomy. Both procedures describe a four port technique used to optimize the cosmetic outcome.
