Hepatectomy in the Setting of Abernethy Malformation

Case Description: A young female presented with abnormal liver function tests and multiple liver masses. Imaging demonstrated a left liver lesion as well as absence of a portal vein. After a hepatectomy, a hepatic adenoma as well as portal vein structures replaced by fibroblastic changes were found. Discussion: This case demonstrates liver resection in a patient with a very rare abnormality - Abernethy malformation, or congenital absence of the portal vein. Non-invasive imaging identified this pre-operatively and surgical resection was able to be performed safely. This patient will remain high risk for developing subsequent liver lesions and will require continued follow-up and screening.
