Iatrogenic Surgical Achalasia: a Mesh Complication After Hiatal Hernia Repair

The use of mesh during hiatal hernia repair is controversial . While the use of synthetic mesh for the repair is associated with a lower rate of recurrence if a complication of the mesh occurs it is frequently catastrophic and often requires an esophogastrectomy. We present a patient who underwent laparoscopic paraesophogeal hernia repair with postoperative dysphagia. He had a second procedure with attempted mesh removal that was aborted. When he presented to us he had solid food dysphagia and a sigmoid esophagus. We were able laparoscopically to remove the mesh and reduce and takedown the incarcerated wrap. At six months the esophagus has regained a more normal shape and function. The patient is asymptomatic
