Laparoscopic Nissen Fundoplication: 5 Year Follow-Up

Introduction: There are few reports in the literature on outcomes 5 or more years following laparoscopic Nissen fundoplication. Methods: GERD specific quality of life questionnaires (QOLRAD), short form health surveys (SF12), and queries regarding current medication use and long term satisfaction were mailed to all patients undergoing laparoscopic Nissen fundoplication at our institution. Results: Results are reported as mean?SEM. Seventy six patients underwent laparoscopic Nissen fundoplication between November 1992 and December 1997. Completed questionnaires were returned in 50 patients (66%). Mean follow-up was 5.1?0.2 years (range 4-9). 88% of patients were satisfied with the long-term outcome and would have the procedure again. Only 22% of patients were on routine anti-secretory medications (18% proton pump inhibitors and 4% H2-receptor antagonists). QOLRAD scores were high (5.8?0.2, scale 0-7, a higher score reflects improved QOL) and compare with that in the general population (6.0). SF12 mental and physical scores were good as well (46.6?1.7 and 34.2?1.6, respectively, versus 50.7 and 51.2 for the general population). Conclusion: Laparoscopic Nissen fundoplication is an effective long-term treatment for gastroesophageal reflux disease. It results in good control of GERD symptoms with high patient satisfaction, improved quality of life, and elimination of anti-secretory medications in the majority of patients. |
